Membership Application Form

There are two ways to submit your CILT Membership Application Form:

(1) Online Submission:

To fill up the Membership Application Form online, please save the file in your computer

then fill in all required information and click submit at the bottom of the pdf file.

Note: Your email application (MS Outlook, Outlook Express, Internet Email) should be open if you wish to do the online submission.

(2) Print and Send:

Download & print the blank Application Form

Please write legibly as you fill in the form and print. You may scan and send it by e-mail to cilt@haward.org.
